Medical/Parenting/Family Emergency/Military Leave of Absence

Leave of Absence Requirements

  • Submission of completed Leave of Absence Petition
  • Receipt for $20 non-refundable petition fee (paid at Cashier’s office 1212 SAASB)
  • For a Medical Leave:
    • Note* from a licensed physician/psychiatrist (M.D., D.O., D.P.M.) or licensed counseling or clinical psychologist (PhD, PsyD) that includes:
      • A recommendation for a leave of absence from studies due to medical issues (specific medical details are not required)
      • The amount of time recommended for the leave of absence
      •  Notes from chiropractors, acupuncturists, or nurses will not be accepted
      • *Please note that all doctor's notes are destroyed after the leave of absence is approved
  • For a Pregnancy/Parenting Leave:
    • For a pregnancy-related leave, a note* from a licensed physician that recommends a leave of absence from studies
    • For parenting, either the birth certificate* or verification from an adoption or similar agency
    • *Please note that all doctor's notes/birth certificates are destroyed after the leave of absence is approved
  • For a Family Emergency Leave:
    • Explanation of circumstances  on the petition or attach a separate page
    • International students are required to meet with an OISS representative before requesting a Family Emergency Leave of Absence
  • For a Military Leave:
    • Documentation from the government of the home country
